For many, heart failure and/or COPD can be difficult to manage with the increased chance of hospital readmission and other complications. The Heart Failure/COPD Clinic is a resource for you and your family members. You will receive personalized education about the condition you have and when appropriate, be given treatment under guidance of your physician.

What to Expect

With a physician’s order, the Palos Community Hospital Heart Failure/COPD Wellness Clinic provides patients with:

  • One-on-one, disease specific education tailored to your needs with either an Advanced Practice Nurse or a Respiratory Therapist as appropriate
  • A focused physical assessment
  • Ability to receive treatments or medications at the time of the visit
  • Assistance in making follow-up appointments with your physician

Additional resources available include speaking with a clinical pharmacist to provide medication education as well as a clinical dietitian for dietary advice specific to your condition.

Some patients benefit from one visit while others benefit from additional visits. The clinic may serve as an ongoing resource for you and your physician with the goal of managing your condition.
