This program includes hands-on instructions about the self-monitoring of blood glucose, education about diet and exercise to both treat and prevent diabetes, development of an insulin treatment plan developed specifically for the patient who is insulin-dependent, and motivation for patients to use the skills for self-management and make any necessary lifestyle changes.
The team approach of our Diabetes Program means that you'll be learning diabetes self-management skills from our dedicated team of certified diabetes educators, registered dietitians, registered nurses, licensed pharmacists, registered physical therapists and licensed social workers.
