Palos Community Hospital Home Health and Palliative Care, Private Duty, and Hospice provide quality personal care and support services to patients who are chronically ill, those with functional impairments, persons needing specialized skilled care at home, those recovering from a hospital stay and the terminally ill.
Through Home Care, patients in many stages of recovery can benefit from our highly individualized care plans and comprehensive range of services. We provide continuity of care for patients newly discharged from the hospital or an extended-care facility so they may return home sooner. Similarly, with appropriate home health care, many ill or disabled patients are able to avoid hospitalization entirely.
Our Hospice program commonly provides services to patients with end-stage cancer, as well as cardiac, renal, lung, liver and nervous system diseases, including those with Alzheimer's, Parkinson's disease, end-stage stroke and multiple sclerosis.
