Who is eligible for Home Care?
Patients in many stages of recovery can benefit from our highly individualized care plans and comprehensive range of services.
We provide continuity of care for patients newly discharged from the hospital or an extended-care facility. In fact, the comprehensive nature of our therapeutic services often allows such patients to return home earlier than might otherwise be possible.
Similarly, with appropriate home health care, many ill or disabled patients are able to avoid hospitalization entirely by receiving the specialized care they need in the comfort and privacy of their own homes.

Who pays for Home Care?
The cost of Home Care often is fully covered by major commercial and private insurance plans, as well as Medicare and Medicaid. Upon admission, we will discuss with you and your family how the services will be covered, and whether you will have any financial responsibility.

Home Care Frequently Asked Questions
My mother was diagnosed several years ago with Alzheimer's Disease. We are noticing some changes, but really aren't sure what we should be doing. Is there something that Home Care can do to help us?
Your mother may or may not qualify for Home Care Services. Medicare and most other payers have qualifying criteria for individuals to receiver covered Home Care services. You can call us at (630)257-1111, and one of our nurses will be able to answer your questions and point you in the right direction.
My dad is in another hospital recovering from surgery. Can we still use Palos Community Hospital Home Care once he comes home?
Yes, Medicare patients retain the right to choose any provider of home care. However, some private payers have contracts with specific home care agencies. You can give us a call at (630) 257-1111, and we will notify that hospital and facilitate the referral to Palos Community Hospital Home Health Care.
My mom is recovering from heart surgery, and is unable to drive herself to doctor's appointments and therapy. Would Home Care be able to help?
Yes, in addition to all of the services offered through Home Health Care, our post-op heart surgery patients are placed on a Home Telemonitoring System. A small monitor is placed in the patient's home. On a daily basis, the patient measures their blood pressure, pulse, weight and oxygen levels. The information is relayed to a computer in the Palos Home Health Care office to a nurse who monitors the results and contacts your physician if there are any irregularities. This service also is available for all Home Care patients, and patients not enrolled in Home Care on a private pay basis. Simply call us at (630) 257-1111 and ask for information on telemonitoring.
What if I need help on the weekends or after 5 p.m.? Does Home Care provide those services?
Yes, a registered nurse is available 24-hours a day, 7-days a week for our Home Care and Hospice patients.
My mom is probably more appropriate for Hospice, but she does not want Hospice. Can Home Care continue to care for her as her condition worsens?
Yes, a team of registered nurses, social workers and chaplains in Home Care have advanced training in Palliative Care. This program is designed for patients who are not quite ready for Hospice, who are perhaps pursuing active treatment but require pain and symptom management, and spiritual and/or psychosocial support. My wife suffers from severe depression. Are there mental health nurses available through Home Care?
Yes, a team of nurses with years of experience with patients with mental health disorders, dementia, Alzheimer's and a variety of other illnesses are available to assist patients and families in achieving the highest quality of care.
