All of our staff members have received special training in Home Health and Hospice care, which includes instruction by a registered dietitian, physical therapist, hospice nurse and an Alzheimer's specialist. We use a team approach to patient care whereby our team of Home Health Care specialists work with the patient, family and physicians to create an individualized care plan, and emotional, spiritual and practical support is given based on the patient's wishes and the family's needs.

Home Health Care and Hospice services are available 24 hours a day, seven days a week to residents of Cook, Will, Kendall and DuPage counties. Patients are never refused on the basis of race, color, sex, religion, national origin or ancestry, handicap, sensory impairment or age.
Our programs are accredited, state-licensed and Medicare-certified.
All components of the Palos Community Hospital Home Health program are accredited by the Joint Commission on Accreditation of Healthcare Organizations (JCAHO), which means we have undergone rigorous scrutiny of our programs and practice.
