Patient Navigator
As a special service to our breast health patients, Palos Community Hospital offers the assistance of a specially trained registered nurse to educate and advocate for you and your family during diagnostic evaluation, treatment and recovery.
Our Breast Health Patient Navigator knows that your well-being and peace of mind are as important as your treatment, and she will serve as a liaison between you, your doctors and your family.
What Does a Breast Health Patient Navigator Do?
- Educates you on breast health issues.
- Explains tests and procedures, including biopsies.
- Helps you navigate our health care system if a breast cancer diagnosis is made.
- Explains treatment options, including surgery preparation, radiation and chemotherapy.
- Assists you in making informed decisions about your care.
- Coordinates appointments with medical team members and facilitates communication with physicians.
- Ensures that follow-up care efforts are made.
- Identifies appropriate social services in your area, including local resources and support groups.
- Supports you and your family throughout the process.
Patients who have the support of a Breast Health Navigator through treatment are better able to focus on healing. More importantly, they can be assured that they are not alone and that an advocate is there to help guide them every step of the way, from diagnosis to treatment and recovery.
